Double-click the My Computer icon on your desktop. Double-click
the Dial-Up Networking icon. The Dial-Up Networking folder
will now open.
Double-click the Make New Connection icon. The Windows
95/98 Make New Connection Wizard will open and guide you through
the rest of setup process.

Now you will see the newly created icon in the Dial-Up Networking
folder named SYSCO
Click with the right button on the icon sysco in the Dial-Up Networking
folder and click Properties.

Click TCP/IP Settings and fill as below.

Click OK to save settings.
Final seting: your name & password. In Dial-Up Networking
folder duble-click on the SYSCO icon. The Connect To
window will open, and you must fill the User name and Password
fields with your own.

Click Connect to make the connection.
At this point, your modem should dial at SYSCO and the connection
will be established.
